The Community Alternatives Program for Children (CAP/C) is a waiver service program for children under 21 years of age who are medically fragile or are at risk for long-term care or hospitalization due to a medical illness or condition. CAP/C provides case management/care coordination services and other supports to these children and their families so that they can live at home within their natural environment.
Jordan Management Group, LLC serves as a lead CAP/C Case Management Agency for the following counties:
Greene, Hertford, Hoke, Hyde, Martin, Northampton, Onslow, Orange, Pamlico, Pasquotank,
Perquimans, Person, Pitt, Rockingham, Tyrrell, Wake, and Washington.
